Ayushman Card is a part of Pradhan Mantri Jan Arogya Yojana (PMJAY). This scheme was launched in 2018. It aims to provide cashless treatment to poor and vulnerable families. This card allows free treatment in any listed government or private hospital.
The central government has launched the Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (PM-JAY) to provide free treatment to low-income families. Under this scheme, eligible citizens get free treatment up to ₹5 lakh per year. The Uttar Pradesh government has recently launched a state-wide Ayushman Card campaign. People can now make cards online from the comfort of their homes, and special camps have also been set up in every district. In this way, the convenience of Ayushman card is being ensured to the common people.

Step 1: First, go to the Ayushman Bharat Beneficiary Portal and select the Beneficiary option.
Step 2: Enter your Aadhaar linked mobile number and captcha, then click on Generate OTP. Enter the received OTP to login.
Step 3: Now select your state and district and confirm eligibility by entering your Aadhaar number. Family and card status will appear here.
Step 4: If “Not-Generated” appears, click Apply Now.
Step 5: Enter your name, date of birth, and family details, and upload the required documents.
Step 6: Verify the Aadhaar OTP, check all the details, and submit the application.

- Ration card
- Proof of income
- Employee Registration Certificate (if applicable)
- Aadhaar linked mobile number
- After this process, your application will be completed, and your Ayushman card will be ready in a short time.
